            Outcome of Annual Evaluation
            The outcome of the evaluation should summarize findings regarding services in scope and quality, and may also
            include:
               •   Whether the health and assistance needs of the service area/population are being met (based upon requests
                    for services and the acceptance of clients/patients).
               •   Whether there is sufficient qualified personnel available to provide the services offered.
               •   Evaluating the extent of cooperation and communication with other agencies, community interest groups,
                   and the effective use of community resources.
               •   Determining the extent that the agency’s policies and procedures are followed.
               •   Need for future planning and methods of implementation for new or changing services as needs are identified.
               •   Whether or not the agency has achieved progress toward meeting annual goals.
               •   Whether or not there is a need for change in:
                   •      Services
                   •      Staffing
                   •      Operating hours
                   •      Geographic service area
                   •      Population groups for whom this agency provides services
                   •      System for continuity of care
                   •      Financing of services

            The annual evaluation will be forwarded to the Governing Body for review and approval.  All annual evaluations will be
            maintained separately in the agency’s survey manual.
